Financial assistance is available to help you pay for therapeutic recreation classes and memberships. In some cases, assistance with transportation may also be available.
To apply, a completed financial assistance application and confirmation of MS diagnosis from your doctor is required. Amount of financial assistance is derived from a sliding scale based on the information from the financial assistance application. Download the forms below or call the Chapter at 1-800-344-4867.
Applications are accepted year-round, however completed applications will only be approved during the following months:
